Petition for Name Change

This is the document you will take to the courthouse! The form asks you to provide a reason for your name change. The judge will read this aloud in court. I used "to affirm my gender identity" and that was sufficient.

Passport Renewal Application

This form (the DS-82) is for requesting a new passport. Fill it out with your chosen name and gender markers. Ensure to check Yes at the bottom of Section A to indicate that your name has changed, and check Yes in item 3 to indicate that your gender marker has changed.
Name changes require submission of a signed court order, which we will obtain soon! Gender marker changes do not require any additional documents. The available gender markers are M, F, and X.

Driver's License Change of Gender Designation Request

This is a simple form for changing the gender marker on your Washington driver's license. You don't need any supplemental documents! Simply bring the completed form to your local Department of Licensing Office or mail it to the address on the form. The available gender markers are M, F, and X.
